Find the Care Gaps and HCCs Your Plan Is Missing

Two minutes to request. No obligation, just an independent, chart-level view of your quality and coding accuracy.

Open HEDIS gaps cost Star Ratings and quality bonuses. Unsupported HCCs create RADV exposure, and missed conditions understate member risk. Our free review samples your charts and shows exactly where each is happening.

Built for health plan quality and risk adjustment teams

HIPAA Compliant BAA and NDA Before Any Data Credentialed Abstractors and Coders Two-Way HCC Review

How We Help Health Plans

Valiant Lifecare supports health plans with HEDIS medical record review, chart retrieval, and retrospective, prospective, and concurrent risk adjustment coding, delivered by credentialed teams under HIPAA-compliant controls.

Close Care Gaps

We find hybrid measure evidence that is in the chart but not captured, so it counts toward your HEDIS rates.

Reduce RADV Risk

We flag submitted HCCs that the documentation does not support, before an audit does.

Capture True Risk

We identify documented conditions that were never coded, so member risk scores reflect reality.

What You'll Walk Away With

Frequently Asked Questions

Is the gap review really free?

Yes. There is no cost and no obligation. We review a sample of member charts and coded encounters and share what we find, including measure-level and HCC-level observations.

What does the review cover?

Two things: HEDIS, where we look for open care gaps and hybrid measure documentation that is in the chart but not captured, and risk adjustment, where we check whether submitted HCCs are supported (MEAT) and whether documented conditions were missed, under the CMS-HCC V28 model.

What do we need to provide?

Just your name, work email, phone number, and plan name to begin. A specialist then agrees the sample size and arranges secure, HIPAA-compliant access to charts or a data extract, under a Business Associate Agreement and NDA.

How long does it take?

You hear from a specialist within one business day. Most reviews are completed within two to three weeks of data access, depending on sample size and chart retrieval.

Is this two-way coding?

Yes. We report both adds (documented, uncoded conditions) and deletes (submitted HCCs the documentation does not support). Two-way review is the defensible approach for RADV exposure.

Who is this for?

Medicare Advantage, ACA marketplace, and Medicaid managed care plans, plus risk-bearing provider groups and ACOs that want an independent view of quality and coding accuracy.
